Include information reasonably sufficient for us to contact you, such as your name, organization, mailing address, telephone number, and email address.
Explain why you believe the identified use infringes your rights and provide any registration details, ownership information, authorization documents, or other supporting material that may help us evaluate the claim.
Where applicable, include a statement that you have a good-faith belief that the disputed use is not authorized by the rights holder, its agent, or applicable law.
Include your physical or electronic signature and identify whether you are the rights holder or a person authorized to act on the rights holder's behalf.
Accuracy Matters

Enforcement
